Four people were injured in the accident which shut down Highway 5, 11 kilometres west of Humboldt, for about five hours on Wednesday.
When RCMP arrived the lone occupant of the westbound vehicle was still trapped inside and the three people in the eastbound vehicle were standing outside.
EMS and Fire from Humboldt extricated the man trapped who was then flown by STARS to Saskatoon Hospital. The other three people were also taken to hospital by ambulance.
Both drivers of the two vehicles, and a man who was a passenger, are in Saskatoon hospital with serious injuries. A teenage boy has been released from hospital.
The accident is still under investigation but RCMP say highway conditions were dry and clear at the time of the accident and alcohol and drugs do not appear to be a factor.
